Client Checklist/Release of Liability. Please read and initial under each statement and submit with Furniture Ministry Referral Form. Our mission of the First Presbyterian Furniture Ministry is to provide good, useful furnishings to those in need, treating each client graciously with Christian hospitality.

I can receive furniture from First Presbyterian Church Furniture Ministry only once within a five-year period. After I receive furniture, my case is closed and additional furniture may not be requested. *

I will notify the Furniture Ministry by calling 336-212-2937 if my address or phone number changes or my household receives any of the requested furnishings. *

Due to limited supply of furniture and high demand, if items I requested are not available at the time of my scheduled pick up, I cannot request or receive them at a later date. *

I am accepting these items “As Is” without warranty or guarantee. *

Furniture Ministry has no way of knowing if these items are complete with all the original parts or that they work as originally intended. *

Furniture Ministry has no way of knowing if these items are up to current industry, legal or fire codes.

Furniture Ministry cannot guarantee that these items are safe or appropriate for children. *

Furniture Ministry makes every effort to inspect each piece, but cannot guarantee there no bugs or pests. If the services of an exterminator become necessary, I understand I will bear the costs of such service. *

Furniture Ministry is not responsible for any damage or injury caused by these items. *

I cannot exchange or return items.

Information about my case may be shared between the Furniture Ministry and the referring agency. Initialing here gives the Furniture Ministry permission to share and receive information pertinent to my case with the referring agency. *
